Your first visit to one of our offices establishes a vital foundation for our doctor-patient relationship. During this visit we will gather the important information such as your medical history, and give you ample time to get to know your Podiatrist, and have the doctor answer your questions.
You can even save some time by printing out and completing the forms on the bottom of this page prior to your first appointment. Please come 15 minutes early to fill out the forms if you are not bring them already filled out. Also please have your insurance card as well as a picture ID with you at time of first visit.
Please check the list of our accepted insurances below. This is not a complete list, so double check with our staff when making the appointment. We look forward to your first visit with us, knowing that we will exceed your expectations and alleviate your discomfort. If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before scheduled appointment time.

INSURANCE INFORMATION
We accept Medicare and its supplements and most private (PPO) insurances. Please call our office for verification of your specific insurance plan and benefits. Our list of insurance includes but is not limited to BCBSIL, Medicare and it’s supplements, Humana, Great West, Cigna, United Healthcare, Professional Benefits Administration, Multiplan, Workman’s Compensation. We accept all major credit cards, cash and check payments, and offer significant discounts to cash payments.
